Answer

While it is not possible to confirm the specific reason in every case, the most common reasons why a one-year health review may not have been completed include:

•Family relocation - movement out of Trust / NI

•Parental Choice to decline the review

•Repeated non engagement despite follow up attempts

•Vacant Caseloads – Staff capacity

Quarterly reports identify outstanding year 1 health reviews, and a range of follow up processes are in place. Additional weekly/ weekend clinics are offered to families of children with outstanding year 1 health reviews to minimise the risk of children being overlooked.

Where there is parental decline or persistent non engagement, appropriate safeguarding policies and procedures are followed in line with Trust & regional guidance. Health Visitors will also liaise with GP where appropriate.
